Skilled Nursing
St Vincent Home Health Care Inc. California-certified and licensed Registered Nursing team partner with physicians and family members to provide optimal care and a safe recovery for every patient’s unique need. We make it our goal to consult our patients for feedback and suggestions and we focus on being reliable, thorough, competent and inviting. Our post-acute care team of Registered and Skilled nurses works with the patient, their family members and the primary physician to ensure that the target goals for recovery and rehabilitation are achieved while still tending to the patient’s comfort and creating a nurturing environment. Additionally, our nursing staff train the patient, their family members and other caregivers on specific in-home care instructions and are available for consultations if any questions or concerns arise.

Services:

Collaborate with the primary doctor and medical team to implement and facilitate the patient’s Plan of Care -Provide disease management tools training, potential complications, medication instructions or learning to cope with a recent diagnosis

Provide pain management relief, wound care, vital sign monitoring and diabetic care including medication management, dieting, exercise and assisted device strength training

Educate patients on self-care after the following procedures: tracheotomy, urostomy care, colostomy and ostomy
Ensure the patient is in a safe living environment without infringing on the patient’s rights or privacy.

Physical Therapy
St Vincent Home Health Care Inc. Physical Therapy team promotes wellness, healthy lifestyle choices, and injury prevention. We help patients maintain their physical health and self-awareness to achieve independent mobility. By following each patient’s physician-prescribed care plan and employing proper educational methods, we prevent further deterioration. Our physical therapists assist a diverse range of patients, from those recovering from surgery or injury to those needing geriatric care, offering services from brief check-ins to round-the-clock coverage. Together, we address health challenges and work toward restoring the highest possible level of independence, both at home and in the community.

Occupational Therapy
St Vincent Home Health Care Inc Occupational Therapy team helps patients recover from or adapt to disability and/or illness. The Team works collaboratively with the patient, their family members and the primary physician to create and carry out the plan of care which is intended to guide patients’ in their adjustment period; this ultimately leads them to confidently resume ownership of their daily lives. More specifically, the Team incorporates exercises involving everyday tasks and routines to assist patients in their rehabilitative progress.

Medical Social Workers
St Vincent Home Health Care Inc. Medical Social Workers team hold graduate degrees in Social Work with extensive medical experience. We strive to enhance and maintain a patient’s psychosocial and physical well-being. More specifically, the team concentrates on health issues such as: providing counseling to patients and their families on in- home health care policies and resources, explaining additional treatment options that may be available and offering guidance on illness and trauma recovery and their mental and physical consequences. Our social workers facilitate communication between patients and their medical team so that patients’ concerns and questions are addressed.
What Do Medical Social Workers Do?

Manage the coordination of meals and transportation after a hospitalization or discharge from a care facility to a home or other care facility

Adapt treatment plans as necessary to adjust to changes in a patient’s status and doctor’s recommendations. Monitor, evaluate and record patient progress according to the treatment described in the Plan of Care.

Collaborate with the primary physician, family members and other medical team members to develop a Plan of Care to assess a patient’s medical or physical condition and their needs.
Refer and counsel patients and family members in community resources and support groups which help in overcoming dependencies and adjusting to new daily routines.

Speech Therapy
St. Vincent Home Health Care Inc. Speech Therapy team specializes in developing treatments to improve overall communication and swallowing abilities. A Speech Language Pathologist will provide an initial consultation to the patient, assess and diagnose specific communication problems and follow the prescribed plan of care in order to rehabilitate basic communication skills. Furthermore, Health Link specializes in difficulties in feeding and swallowing, as well as, offering a variety of therapeutic options to rehabilitate swallowing disorders. Our treatment goal is to have our patients attain an enjoyable, safe and satisfying daily routine in their eating and drinking habits.
